Dual amlodipine/atorvastatin tx

- Start: 5 mg/10 mg-5 mg/20 mg PO qd; Max: 10 mg/80 mg/day; Info: start 2.5 mg/10 mg-2.5 mg/20 mg PO qd in small or fragile pts or pts 65 yo and older; start 2.5 mg/40 mg-5 mg/40 mg PO qd in pts w/ >45% LDL reduction goal; refer to individual drugs for indication-specific dosing
