Dose: 750 mg IV x2 doses >7 days apart; Max: 1500 mg/tx course; Alt: 15 mg/kg/dose IV x1, max 1000 mg/tx course; Info: for pts w/ non-dialysis dependent CKD or unable to take PO form; may repeat tx course if recurrent iron deficiency anemia
