Acute coronary syndrome
- [2 mcg/kg/min IV]
- Start: 180 mcg/kg/dose up to 22.6 mg IV x1; Max: 15 mg/h infusion; Info: give until discharge or CABG surgery up to 72h duration; if undergoing PCI, give until discharge or x18-24h post-procedure up to 96h duration; may use w/ aspirin, heparin
Percutaneous coronary intervention

- Start: 180 mcg/kg/dose IV up to 22.6 mg x1 before procedure; Max: 15 mg/h infusion; Info: repeat bolus dose x1 10min after initial dose; give infusion for at least 12h up to discharge or x18-24h post-procedure; may use w/ aspirin, heparin
