Recommended Adult Dosing

Dosage forms:  ER TAB: 82.5 mg, 165 mg, 330 mg

Neuropathic pain, diabetic

[165-330 mg PO qd]

Start: 165 mg PO qd, may incr. to 330 mg PO qd w/in 1wk; Max: 330 mg/day; Info: give after evening meal; do not cut/crush/chew tab; taper dose over >1wk to D/C

Post-herpetic neuralgia

[165-330 mg PO qd]

Start: 165 mg PO qd, may incr. to 330 mg PO qd w/in 1wk, then may incr. up to 660 mg PO qd after another 2-4wk; Max: 660 mg/day; Info: give after evening meal; do not cut/crush/chew tab; taper dose over >1wk to D/C

Renal dosing

CrCl 30-60: decr. usual dose by 50%; CrCl <30: avoid use
