Info: for pts w/ hematologic malignancy receiving autologous HSCT conditioning regimens predicted to result in Grade 3 or 4 mucositis; admin. 3rd dose 24-48h before start of myelotoxic tx; admin. 4th dose on day 0 after completion of hematopoietic stem cell infusion, at least 7 days after the 3rd palifermin dose
