Dosage forms:  TAB: 240 mg, 480 mg; INJ: various

CMV prophylaxis, allogeneic HSCT

[480 mg PO/IV qd until 100 days post-transplant]
Start: between days 0 and 28 of transplant; Info: for CMV seropositive recipients; switch to PO form when possible
