 Recommended Adult Dosing

Dosage forms:  SOL: 50 mg per 5 mL

UTI, uncomplicated

[100 mg PO q12h x10-14 days]

Alt: 200 mg PO q24h x10-14 days

UTI prophylaxis

[100 mg PO qhs x6-24wk]

Info: not 1st-line tx; high recurrence rates due to TMP-resistance

PCP tx, mild-mod.

[15 mg/kg/day PO divided tid x21 days]

Info: give w/ dapsone

Renal dosing

CrCl 15-30: decr. usual dose by 50%; CrCl <15: avoid use

HD: 100 mg q24h, on dialysis days admin. after dialysis; consider supplement if next maint. dose not due right after dialysis; PD: 100 mg q24h; no supplement
