 Recommended Adult Dosing

Dosage forms:  ER TAB: 25 mg, 50 mg, 100 mg

Major depressive disorder

[50 mg PO qd]

Info: no proven additional benefit at doses >50 mg/day; do not cut/crush/chew/dissolve tab; taper dose gradually to D/C

Neuropathic pain, diabetic

[200 mg PO qd]

Start: 50-100 mg PO qd, incr. dose q3 days; Info: do not cut/crush/chew/dissolve tab; taper dose gradually to D/C

Renal dosing

CrCl 30-50: max 50 mg/day; CrCl <30 or ESRD: max 25 mg/day or 50 mg qod

HD: max 25 mg/day or 50 mg qod; no supplement after dialysis; PD: max 25 mg/day or 50 mg qod; no supplement

Hepatic dosing

mod-severe impairment: max 100 mg/day
